Nebraska Utilizes The Prevention Plan CMâ„¢ to Relieve Pressure on Medicaid

U.S. Preventive Medicine®, through its wholly-owned subsidiary U.S. Care Management®, has been selected by the Nebraska Department of Health and Human Services to implement a new community-based disease management program for eligible Medicaid recipients with multiple chronic medical conditions. U.S. Preventive Medicine® will utilize The Prevention Plan CM, its highly-regarded field-based chronic disease management program, for The Enhanced Care Coordination Program in Nebraska.

The program will offer innovative approaches for providing disease and care management services to high-risk Medicaid patients who are living with multiple chronic medical conditions including asthma, cancer, diabetes, congestive heart failure and hypertension and others. 

Registered nurse care managers will work with participants face-to-face and telephonically to offer education, care coordination and support. The goal is to help clients change behavior and more closely adhere to treatment plans, ultimately improving their health and reducing hospitalizations.

RN Care Managers complete a thorough assessment, including medical, social and behavioral issues to better understand the client’s needs and uncover any barriers to compliance. Throughout the program, the nurses help clients understand their often complex treatment plans, encourage lifestyle improvements, and coordinate care between providers and various community services. A 24-hour, seven-day-a-week toll free nurse line is also available for individuals to access information.

A year ago, Nebraska spent more than $80 million in state and federal dollars for the care of Medicaid recipients, whose bills for the year totaled at least $50,000 each. Based on national data, Nebraska could see a savings of $1.30 for every dollar spent on The Enhanced Care Coordination Program.

U.S. Care Management® is accredited in disease management by URAC, an independent, nonprofit organization that is well-known as a leader in promoting health care quality through its accreditation and certification programs. U.S. Preventive Medicine® has provided its community-based services through 21 programs with over 300,000 eligible participants in 13 States: Arizona, Arkansas, Colorado, Florida, Mississippi, Montana, Nebraska, North Dakota, New Hampshire, New York, Oregon, Texas and Washington.
